As a Social Prescribing Specialist, you will act as a bridge between healthcare and the wider community. You will work closely with patients to understand their needs and goals, then develop personalized care plans that incorporate social prescribing interventions. This may involve connecting patients with local groups, activities, or services that address their specific challenges, such as loneliness, financial hardship, or mental health concerns.
Key Responsibilities:
- Conduct comprehensive assessments to identify patients' social needs and support requirements.
