We are seeking a highly skilled and motivated Independence Support Worker to join our team at Bedfordshire Hospitals NHS Foundation Trust. As an ISW, you will play a pivotal role in promoting reablement and well-being across all medical wards, working closely with nursing and therapy staff to support patients in maintaining their pre-admission mobility, function, and well-being.
Main Responsibilities
The successful candidate will be responsible for:
• Conducting assessments and developing treatment plans to support patients in achieving their goals
• Providing individualized support and care to patients, including assistance with daily living activities, mobility, and transfers
• Collaborating with multidisciplinary teams to ensure comprehensive care and support for patients
• Maintaining accurate and up-to-date patient records and reports
• Participating in ongoing education and training to enhance skills and knowledge
Requirements
To be successful in this role, you will need:
• A strong understanding of reablement and rehabilitation principles and practices
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
