About the Role

We are seeking a highly skilled and compassionate Support Worker to join our Integrated Outreach Team. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with psychosis and complex needs.

Key Responsibilities
  • Work collaboratively with the Integrated Outreach Team to deliver person-centered care and support
  • Develop and maintain strong relationships with service users, their families, and other stakeholders
  • Contribute to the development and implementation of individual care plans
  • Provide emotional support and guidance to service users
  • Participate in regular team meetings and contribute to the development of the team
Requirements
  • Experience of working in a care or support role, preferably in a community setting
  • Understanding of the role of the Integrated Outreach Team and the needs of service users
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Flexibility and adaptability in a fast-paced environment
