Position Overview
An exceptional opportunity has emerged for a skilled prescribing Pharmacist to join the team as a Clinical Pharmacist within a Primary Care Network (PCN).
This role is centered in practice, where the successful candidates will collaborate with multiple practices, concentrating on elderly patients experiencing frailty (multi-morbidity) and individuals with chronic conditions such as cardiovascular diseases, asthma/COPD, and diabetes. The ideal candidate will also assist practices in enhancing the management of atrial fibrillation through suitable anticoagulation, monitoring high-risk medications, and ensuring prescribing safety in accordance with the new GP contract.
Key Responsibilities
The successful candidate will integrate into a multi-disciplinary team in a patient-focused capacity, taking charge of chronic disease management within the practices and conducting clinical medication reviews to effectively manage patients with complex polypharmacy. They will also serve as the primary resource for general practice staff regarding prescription and medication inquiries.
The Clinical Pharmacist will be expected to cultivate productive working relationships both internally and externally to achieve the high standards set by the practice. In return, the successful candidate will assume a role that is strategically positioned to positively impact patient care while being part of a team dedicated to staff development.

This organization aims to coordinate activities around patient groups requiring ongoing care while continuing to provide standard GP services for our patients. We are committed to focusing on population health and supporting preventive initiatives, including vaccination programs.
Job Responsibilities
Role Scope
The post holder is a pharmacist who operates within their professional boundaries, supporting and collaborating with a team of pharmacists in general practice.
The post holder will engage in a patient-facing role as part of a multi-disciplinary team. They will oversee chronic disease management within the practice and perform clinical medication reviews to proactively manage patients with complex medication regimens.
Key duties include:
- Conducting patient-facing long-term condition clinics.
- Assessing patients with single or multiple medical issues requiring medication optimization.
- Reviewing the ongoing necessity for each medication and monitoring needs while supporting patients in effectively using their medications.
- Making appropriate recommendations to Senior Pharmacists or GPs for medication improvements.
- Performing clinical medication reviews and producing recommendations for senior clinical pharmacists, nurses, and/or GPs on prescribing and monitoring.
- Collaborating with care home staff to enhance the safety of medication ordering and administration.
- Managing common/minor/self-limiting ailments while adhering to a defined scope of practice.
- Providing telephone support for patients with medication-related queries.
- Implementing systems to ensure continuity of medication supply for high-risk patient groups.
- Contributing pharmaceutical advice for the development and implementation of new services.
Qualifications and Experience
Essential
- Minimum of 2 years post-qualification experience.
- In-depth therapeutic and clinical knowledge.
- Excellent interpersonal and communication skills.
- Ability to work effectively both independently and as part of a team.
Desirable
- Postgraduate diploma level or equivalent training/experience.
- Independent prescriber or intent to gain independent prescribing qualification.
