Mental Health Practitioner

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Penwith Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to our patients.
Key Responsibilities- Provide holistic and person-centred primary care mental health services, delivering better access to a broader range of care options and improving patient outcomes.
- Work closely with GPs, primary care colleagues, and other healthcare professionals to develop and implement effective care plans.
- Deliver evidence-based interventions and support patients with serious mental illness, including those at risk of becoming unwell.
- Collaborate with mental health and wellbeing coaches to formulate, plan, and deliver care pathways for each patient.
- Participate in daily huddle/safety meetings and oversee other staff member workloads, ensuring safe and effective service delivery.
- Professional qualification in a relevant discipline, such as nursing, social work, or occupational therapy.
- Experience of working with people with mental illness and those at risk of becoming unwell, including liaison with GPs and primary care colleagues.
- Ability to make decisions autonomously and be accountable for these at this level.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
- Knowledge of prevalent mental illnesses, treatments, and care pathways, as well as an understanding of health and social inequalities experienced by the population of Penwith.
