About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at North Staffordshire GP Federation. As a Care Coordinator, you will play a vital role in supporting our healthcare professionals to deliver high-quality patient care.

Key Responsibilities
  • Coordinate contact with patients to ensure they are aware of available services and support.
  • Manage referrals and monitor patients who have been referred to secondary care to ensure timely follow-up and support.
  • Produce monthly reports on new cancer diagnoses and identify learning opportunities for improvement.
  • Ensure patients receive necessary tests and screenings, such as annual prostate tests and cervical screening.
  • Support patients and GPs with managing issues related to secondary care.
  • Work with the Learning Disabilities team to ensure patients with learning disabilities receive regular reviews and testing.
  • Monitor hospitalizations of patients with learning disabilities and report data to the governance meeting.
  • Organize and participate in Learning Disabilities Death audits and present findings to the clinical team.
  • Collaborate with the LD Care Home Manager and ensure they have access to practice information.
  • Support the Safeguarding team with workflow and document circulation.
  • Summarize and code documents into relevant templates and highlight significant information to patient registered GPs.
  • Maintain a register of children on Child Protection Plans and ensure correct coding on patient records.
  • Add alerts to parent records for children on safeguarding plans.
  • Contribute to data collection and audit for safeguarding to support improvement in standards.
  • Undertake new patient health checks and baseline observations.
  • Support nursing teams with health promotion programs and chronic disease clinics.
  • Assist with answering calls and booking appointments.
  • Support the reception team with covering the reception desk.
  • Liaise with other health professionals to deliver personalized care.
  • Review processes to identify improvements in systems and procedures.
  • Ensure patients on high-risk medications receive regular blood tests.
  • Support vaccination campaigns and work closely with the PCN social prescribing link worker and Mental Health Practitioner.
  • Work proactively with the PCN pharmacist to support medication reviews and regular drug monitoring requirements.
  • Document and code coordinated activity accurately.
  • Build relationships with community service providers.
  • Raise awareness of shared decision-making and decision-support tools.
  • Help patients manage their needs through answering queries, making and managing appointments, and providing written or verbal information to support informed decision-making.
  • Provide coordination and navigation for people and their carers across health and care services.
Requirements
  • Experience working under own direction.
  • Experience in a patient-focused environment.
  • Evidence of experience in a wide range of administrative systems and software programs.
  • Experience of planning and organizing complex meetings and agendas.
  • Evidence of working with IT systems.
  • Evidence of ability to support collation and analysis of data.
  • Excellent verbal and written skills.
  • Ability to provide and receive complex information.
  • Excellent interpersonal skills.
  • Experience of using medical software packages, such as SystmOne and EMIS Web.
  • Proven track record of effective use of networking and influencing skills.
  • Demonstrable ability to show kindness and compassion.
  • Ability to reflect and learn from situations.

Qualifications
