About the Role

We are seeking a highly skilled and dedicated Nursing Associate to join our Community Nursing Team at Southern Health NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities
  • Deliver delegated routine assessments and interventions, spanning the professional domains of nursing, physiotherapy, and occupational therapy.
  • Carry out clinical and welfare checks, concerned for welfare checks, observation monitoring, therapy and physio visits, equipment instillation, and 2-hour response calls.
  • Work collaboratively with other practitioners to provide efficient and effective care to patients.
Requirements
  • Formal Assistant Practitioner qualification, Health & Social Care Degree, or equivalent.
  • Nursing Associate Qualification registered with the NMC.
  • Foundation Degree in Health & Social Care Practice.
  • Care certificate.
Desirable Qualifications and Experience
  • Health & Social Care Foundation Degree or equivalent.
  • Significant experience of working in a relevant health or social care setting.
  • Experience of working in the community.
Additional Criteria
  • Physically capable of performing the role and responsibilities expected of the post holder.
  • Car driver with a clean license.
  • Awareness of personal safety.
