Rochdale Council is seeking a dynamic and skilled Assessment and Support Planning Officer to join our Adult Care Services team. As an Assessment and Support Planning Officer, you will play a crucial role in supporting adults and their carers, promoting independence and well-being, and working towards the objectives of the Care Act 2014.
Key Responsibilities:
- Conduct assessments and support planning for adults, taking into account their individual needs and circumstances.
- Work collaboratively with adults, carers, and other professionals to develop and implement support plans that promote independence and well-being.
- Provide guidance and support to adults and carers on accessing services and resources.
- Contribute to the development and implementation of policies and procedures that support the delivery of adult care services.
- Work effectively with colleagues and other stakeholders to achieve the objectives of the Care Act 2014.
Requirements:
- Experience of working in an adult care setting, preferably with a focus on assessment and support planning.
- Knowledge of strength-based approaches to assessments and support planning.
- Ability to work collaboratively with adults, carers, and other professionals.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and meet deadlines.
