Job Description

We are seeking a highly skilled and compassionate Adult Social Care Worker to join our team in North Somerset. As a key member of our Adult Social Care Locality Team, you will work closely with Deaf and Deafblind individuals, providing support and care to meet their unique needs.

Key Responsibilities
  • Conduct Care Act Assessments and develop care plans with individuals, identifying their strengths and person-centred outcomes.
  • Complete Mental Capacity Assessments and participate in Best Interest Decision making where necessary.
  • Support individuals to identify and build upon their existing assets, as well as access new resources to meet their identified outcomes.
  • Work collaboratively with Health and Social Care colleagues and voluntary sector agencies to provide comprehensive support to Deaf and Deafblind individuals.
Requirements
  • Passion for working with the Deaf community in a strength-based way.
  • Good knowledge of Deaf awareness.
  • British Sign Language (BSL) preferable at level 2 or above.
  • Background or understanding of health and social care or transferable skills related to the job description.
  • IT skills to enable training on various in-house IT systems.
