Care and Case Management Services Ltd (CCMS) is dedicated to delivering exceptional support to our clients and their families, prioritizing their thoughts and expectations in everything we do.
Please note that we cannot offer sponsorship for this role, and therefore, we will not be able to progress any candidates who require a certificate of sponsorship to work in the UK.
We have an exciting opportunity for a motivated and proactive Support Worker to join our support team for a young man with a brain injury living with his family in the Dewsbury area.
You will provide support and contribute to the planning and delivery of individual care, assisting with all aspects of daily living as our client has physical, sensory, and cognitive disabilities and is wheelchair dependent.
Successful applicants will receive comprehensive support, training, and supervision from our established support team and Case Manager.
Key Responsibilities:
- Assist the client in exercising choice and control over their life as far as possible, taking into consideration cognitive problems.
- Liaise with and support the family as required.
- Assist the client with personal care as needed.
- Work with the treating team of therapists (SALT, Physiotherapist, and Occupational Therapist) to complete therapy programs, motivate, and engage the client in recreational and educational activities.
- Support the client in using local amenities, such as shops and leisure facilities, and attending community events.
- Follow the service user plan as documented for the client.
- Maintain accurate and confidential records.
- Participate in moving and handling procedures as specified for the client.
- Maintain professional boundaries with the client and their family members, adhering to direction from the Case Manager as appropriate.
About You:
- Full, clean UK driving license with experience of driving a wheelchair-accessible vehicle (preferable but not essential).
- Experience of delivering therapy programs is desirable, but full training will be provided.
- Dependable and professional, with a strong sense of honesty and discretion.
- Experience of working with young adults with physical and cognitive impairment is essential, and the position is suited to someone with a positive attitude and the ability to motivate and engage.
- Previous experience working with clients with brain injury is preferable.
- Friendly and approachable.
- Successful candidates will be required to undergo an Enhanced DBS and enroll in the DBS update service.
- Flexible and able to work well under pressure.
CCMS is not an employment agency; we help our clients recruit their own support workers as part of our case management service. You will be employed directly by the client, with CCMS overseeing all aspects of the client's care package on their behalf.
CCMS offers a bespoke Case Management Service for people of all ages, with a particular emphasis on those recovering from acquired brain injuries. Our ultimate goal is to empower and support individuals to reach their full potential and maximize their rehabilitation opportunities, promoting the best quality of life.
