Job Description

Remedy Social Work is seeking a highly skilled and dedicated Social Care Specialist to join their Fostering Service team at Knowsley Council.

Key Responsibilities:
  • Support Service Users: Provide high-quality care and support to service users, ensuring their individual needs and service targets are met, while maintaining the highest standards of care and adhering to CQC fundamental standards.
  • Personal Care: Assist service users with their personal care, including activities such as dressing, washing, bathing, or shaving, toileting, getting in or out of bed, eating, drinking, taking medication, and communicating.
  • Monitoring Progress: Regularly monitor the progress of service users, being mindful of triggers for challenging behavior, seizures, and acting appropriately in line with agreed guidelines, while highlighting any changes in an individual's behavior to the manager to update support plans.
  • Emotional Support: Provide emotional support to individuals who may regularly place demands on the jobholder, and raise significant issues to the manager.
  • Activity Planning: Contribute to the planning of daily activities and risk assessments, ensuring they motivate and interest service users, and provide a positive experience.
  • Meetings and Communication: Attend and contribute positively to meetings as requested by the manager, providing input by sharing the outcome of interactions with service users and highlighting issues that may impact the review of the service user's case.
  • Medication Administration: Ensure the accurate administration and recording of medication, including rescue medication, in line with the Council's Medication policy, and follow agreed protocols when supporting an individual's personal care.
  • Record Keeping: Ensure all records are updated and accurate, including care plans, risk assessments, work-related logs, and other template-based documents, while applying appropriate information security protocols when sharing and preparing information relevant to the service user's care.
  • Professional Communication: Act and communicate professionally and appropriately at all times, ensuring direct contact with a range of people involved in care delivery, including service users, families, care/medical professionals, and other colleagues.
