Hunter Gatherer Mental Health is seeking a skilled CPN to join their Adult team in Greater Manchester. As a CPN Care Coordinator, you will be responsible for coordinating, commissioning, and delivering individual packages of care for a caseload of service users within Community Mental Health Teams (CMHTs).

Key Responsibilities:

  • Undertake initial assessments of new referrals to the service.
  • Support the duty system.
  • Coordinate, commission, and deliver individual packages of care for a caseload of service users within CMHTs.

Requirements:

  • Current and valid registration with the NMC.
  • Proven experience within a CPN recovery role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to providing high-quality patient care.
