Job Summary

Lakeside Healthcare Group is seeking an experienced Clinical Pharmacist to join our team in a dynamic and forward-thinking role. As a Clinical Pharmacist, you will play a vital part in improving the quality of care and operational efficiencies within our general practice.

About the Role

The successful candidate will work within their clinical competence as part of a multi-disciplinary team to provide expertise in clinical medicines management. This will involve providing face-to-face structured medication reviews, managing long-term conditions, ensuring safe prescribing at the transfer of care, implementing systems for safer prescribing, repeat prescription reauthorisation, and dealing with acute prescription requests.

Key Responsibilities
  • Provide leadership on quality improvement and clinical audit, and manage some aspects of the Quality and Outcomes Framework.
  • Conduct face-to-face medication reviews of patients with polypharmacy, especially for older people, people in residential care homes, and those with multiple comorbidities.
  • Develop and implement systems for safer prescribing, repeat prescription reauthorisation, and dealing with acute prescription requests.
  • Work with care home staff to improve the safety of medicines ordering and administration.
  • Manage common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
  • Provide patient-facing clinics for those with questions, queries, and concerns about their medicines in the practice.
  • Answer relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams, and patients with queries about medicines.
  • Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
  • Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
  • Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
  • Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients.
Requirements
  • Registration as a Pharmacist with the General Pharmaceutical Council.
  • Evidence of Continuing Professional Development (CPD).
  • Masters degree in pharmacy (MPharm) or equivalent.
  • Independent prescriber (or nearing completion) with experience of managing chronic disease.
  • Postgraduate diploma in Clinical Pharmacy and/or completion of the CPPE Pathway (or nearing completion).
