Job Title: Adult Carers Support Advisor

We are seeking a highly motivated and experienced Support Worker to join our Childrens & Families Team in Bury. As a Support Worker, you will play a vital role in providing practical and emotional support, advice, and guidance to young people aged between 16 and 25 who are experiencing care and leaving care.

Key Responsibilities:
  1. Work directly with young people and their families to safeguard and promote their welfare.
  2. Coordinate Pathway Planning duties to support care leavers in their successful transition to adult life.
  3. Provide support and guidance to young people in achieving their goals and aspirations.
  4. Collaborate with other professionals to ensure a holistic approach to supporting young people.
Requirements:

To be successful in this role, you will need to have:

  • A minimum of 2 years+ UK experience within a Childrens Social Work team.
  • An NVQ3 in Health & Social Care or equivalent.
  • A current DBS.
  • Excellent communication and interpersonal skills.
  • A car driver.
