About the Role:

We are seeking a highly skilled and compassionate Rehabilitation Services Coordinator to join our multi-disciplinary team at Gloucestershire County Council. As a key member of our team, you will play a vital role in supporting individuals with complex needs to remain independent and have a say in how they live.

Key Responsibilities:

  • Provide high-quality care and support to residents, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with healthcare professionals, social workers, and other stakeholders to develop and implement individualized care plans.
  • Administer medication and assist with personal care, ensuring the highest standards of dignity and respect.
  • Lead shifts as the Officer in Charge and supervise Rehabilitation Services Support Workers, providing guidance and support as needed.
  • Contribute to the development and implementation of care policies and procedures, ensuring they are aligned with the council's values and objectives.

Requirements:

  • NVQ Level 3 (or equivalent) qualification in Health and Social Care, or willingness to work towards it.
  • Hands-on experience of working in a care setting, with a strong understanding of care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and sleep-in duties, with a willingness to adapt to changing circumstances.
  • Enhanced DBS check and commitment to safeguarding and promoting the welfare of vulnerable adults.
