Key Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of each resident
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Collaborate with the interdisciplinary team to ensure seamless care delivery and effective communication
- Conduct thorough assessments and risk evaluations to identify potential health concerns
- Document patient progress and maintain accurate records
Requirements:
- Current NMC registration
- Good knowledge of up-to-date clinical practices
- Experience in producing well-developed care plans and detailed risk assessments
- Understanding of regulatory frameworks, including DoLs/MCA and Royal Pharmaceutical guidelines
