About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Nurse to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our liaison team, you will play a vital role in providing high-quality care to patients with mental health needs.

Key Responsibilities
  • Conduct comprehensive assessments of patients' mental health needs and develop individualized care plans.
  • Provide expert consultation and liaison services to clinical teams, ensuring that patients receive the best possible care.
  • Collaborate with medical and nursing staff, families, and carers to ensure seamless care coordination.
  • Develop and implement educational programs for nursing and non-nursing staff, promoting best practices in mental health care.
  • Participate in quality assurance initiatives, contributing to the ongoing evaluation and improvement of our mental health services.
Requirements
  • Registered Nurse (mental health) or Social Worker or Occupational Therapist with current professional registration.
  • Experience in assessing, planning, and evaluating mental health nursing interventions.
  • Proven ability to work effectively in a fast-paced, dynamic environment.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
