About the Role

We are seeking a highly skilled and compassionate Frailty Nurse to join our team at Monkseaton Medical Centre. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to our elderly patients with moderate to severe frailty, multiple co-morbidities, and complex needs.

Key Responsibilities
  • Carry out weekly ward rounds at our aligned Care Home(s) to ensure the best possible care for our patients.
  • Complete Emergency Health Care Plans (EHCPs) and review them as needed to ensure patient safety.
  • Manage the Long-Term Conditions (LTCs) of patients under your care, including completing LTC reviews and managing acute illness.
  • Undertake annual Mental Health, Learning Disability, and Dementia reviews, creating care plans for patients under your care.
  • Communicate with relatives about patient care, including acute and chronic management, and engage in discussions about future care planning.
  • Participate in palliative and End of Life care for patients under your care.
  • Communicate with secondary and tertiary care, referring patients as needed, and participating in health promotion and disease prevention programs.
  • Undertake training of carers and/or relatives to enable them to take and record basic observations, supporting remote monitoring and clinical decision-making.
  • Attend meetings in practice where your contribution will be valuable, including palliative care meetings, audit meetings, and PCN frailty meetings.

  • Ability to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.
  • Ability to use own initiative, discretion, and sensitivity.
  • Able to get along with people from all backgrounds and communities, respecting lifestyles and diversity.
  • Flexible and cooperative.
  • Ability to maintain confidentiality.
  • Problem-solving and analytical skills.
  • Knowledge of and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
Requirements
  • Registered RGN.
  • Adult Nursing degree or equivalent/advanced practice.
  • Experience of using Primary Care clinical system SystmOne.
  • Evidence of working autonomously or as part of a team.
  • Knowledge of national standards that inform practice (NICE guidelines).
  • Awareness of current national and local agenda in health and social care for frailty.
