About the Role

We are seeking a highly skilled and experienced Practice Nurse to join our team at Sleights and Sandsend Medical Practice. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Key Responsibilities
  • Develop and Implement Health Promotion Programs: Design and implement evidence-based health promotion programs to improve patient outcomes and reduce health disparities.
  • Manage Chronic Disease Patients: Provide comprehensive care to patients with chronic diseases, including diabetes, hypertension, and cardiovascular disease.
  • Provide Routine Nursing Care: Deliver routine nursing care to patients, including wound care, immunizations, and travel medicine services.
  • Prescribe Medication: Prescribe medication in accordance with PGD and within your scope of practice.
  • Support the Clinical Team: Collaborate with the clinical team to develop and implement individual treatment plans for patients with chronic diseases.
  • Maintain Accurate Clinical Records: Ensure accurate and up-to-date clinical records are maintained in accordance with legislation and best practice.
  • Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives and participate in the review of clinical practices to ensure they are evidence-based and effective.
Requirements
  • Registered Nurse with the Nursing and Midwifery Council: Hold current registration with the Nursing and Midwifery Council.
  • Experience in Primary Care: Have experience working in a primary care setting, preferably in a practice nurse or community nurse role.
  • Non-Medical Prescriber: Possess a non-medical prescriber qualification (desirable).
  • Clinical Knowledge: Have a broad knowledge of clinical governance, evidence-based practice, and the importance of accurate clinical notes.
