Care Coordinator Opportunity

We are partnering with a Local Authority in Enfield to support the appointment of a Care Coordinator on a 3-month contract, with potential for extension. If you are a skilled professional with experience in care management, we encourage you to apply with your CV for immediate consideration.

Key Responsibilities:

  • Conduct initial assessments within the specialist area.
  • Manage, monitor, and review care plans involving users and carers, liaising with other workers and agencies as necessary.
  • Work within the council's Financial Regulations at all times.
  • Be aware of resources available within the specialist area, including other sources of funding, to ensure maximum benefit for service users within budgetary resources.
  • Utilize management information systems, including information technology, for recording, retrieval, and analysis of information as required by the department.

Requirements:

  • Ability to converse with members of the public in English, providing accurate advice and being polite and courteous when interacting with the public.
  • Ability to assess need and make appropriate support plans, with a good understanding of the care management process.
  • Ability to work in partnership with service users, carers, and other professionals, maintaining the service user at the centre of each process or activity.

Important Notes:

  • You should be available to work immediately or at short notice.
  • You should have the right to work in the U.K.
  • This role requires a basic DBS.
