**About the Role:**

The D2A service provides short-term, high-quality support to individuals who are well enough not to need an acute hospital bed but do need some extra support in their home.

You will be required to assess customers' needs to identify areas where support is needed to promote independent living and provide person-centred care.

**Key Responsibilities:**

  • Assess customers' needs and develop personalized care plans to promote independent living.
  • Provide person-centred care and support to customers with tasks around the home and with personal care.
  • Motivate customers and build their confidence to achieve their goals and return to independent living.
  • Work collaboratively with the D2A team to ensure seamless delivery of care and support.
  • Adhere to Brunelcare's policies and procedures to ensure high-quality care and support.

**Requirements:**

  • Previous experience in caring for vulnerable adults is essential.
  • Sensitive, caring, and passionate about delivering a first-class service to people within your local community.
  • Proactive and self-motivated with great initiative and excellent communication skills.
  • QCF Level 2 in Health and Social Care is essential.
  • A full driving licence and access to your own vehicle are required as you will need to be comfortable with lone working and travelling between customers.
