Key Responsibilities
- Conduct thorough patient assessments and develop individualized care plans to ensure optimal patient outcomes.
- Administer medications and treatments as prescribed by healthcare providers, maintaining accurate records and documentation.
- Monitor patient vital signs and symptoms, and report any changes to the medical team in a timely manner.
- Collaborate with other healthcare professionals to ensure comprehensive patient care, promoting a culture of teamwork and open communication.
- Educate patients and their families on health management and disease prevention, empowering them to take an active role in their care.
- Maintain accurate and up-to-date patient records, adhering to clinical governance standards and best practices.
- Take responsibility for shifts as Nurse in charge, ensuring seamless transitions and continuity of care.
- Manage team members and plan shifts to ensure adequate staffing and coverage.
- Implement infection prevention and control measures to maintain a safe and healthy environment for patients and staff.
- Liaise with General Practitioners and Multi-Disciplinary Teams to ensure coordinated care and optimal patient outcomes.
Requirements
- Patient care experience, preferably in a hospital or clinical setting.
- Experience in a nursing home setting, with a strong understanding of gerontological care.
