We are seeking a highly skilled and dedicated Health Visitor to join our team at Kent Community Health NHS Foundation Trust.
As a Health Visitor, you will play a vital role in supporting the delivery of the Healthy Child Programme, working in partnership with families and other agencies to promote the health and wellbeing of children and young people.
Key responsibilities will include:
- Providing antenatal and postnatal care and support to families
- Conducting universal mandated checks and packages of care
- Working with vulnerable children and their families to identify and address their needs
- Contributing to the development of care plans and ensuring regular communication with caseload holders
We are looking for someone who is passionate about delivering high-quality care and is able to work effectively in a community-based setting.
