Job Overview

Hounslow Community Recovery Service is an integrated health and social care service for adults with complex recovery needs. Our team provides personalized rehabilitation and reablement services to support independence and prevent deterioration in physical, psychological, and social functioning.

Main Duties

This is an ideal opportunity for an experienced Physiotherapist to further develop clinical knowledge and management skills within our multidisciplinary team. The successful applicant will provide therapeutic interventions based on problem-solving and advanced clinical reasoning approaches. Opportunities for in-service and external training will be provided.

Key Responsibilities
  • Provide high-quality physiotherapy services to patients with complex needs
  • Work collaboratively with the multidisciplinary team to develop and implement individualized care plans
  • Conduct thorough assessments and develop treatment plans to achieve optimal patient outcomes
  • Participate in quality improvement initiatives and contribute to the development of evidence-based practice
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Requirements

To be successful in this role, you will need:

  • Bachelor's degree in Physiotherapy or equivalent
  • HCPC registration
  • Postgraduate experience in rehabilitation and management of complex conditions
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
