We are seeking a skilled and compassionate Social Worker to join our North Community Team. As a key member of our team, you will work closely with adults who require support with complex needs, including physical disabilities, mental health challenges, and learning disabilities.

Key Responsibilities
  • Conduct thorough assessments to identify individual needs and develop personalized care plans.
  • Collaborate with service users, their carers, and multi-disciplinary partners to ensure comprehensive support.
  • Manage a caseload, maintaining accurate and timely records of assessments, interventions, and care plans.
  • Participate in safeguarding and mental capacity assessments, ensuring compliance with statutory requirements.
Requirements
  • Relevant social work qualification.
