Mental Health Nurse

About the Role:
Deepdene Care Ltd is seeking a highly skilled and compassionate AD Hoc Mental Health Nurse to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to our residents, working closely with colleagues to ensure the best possible outcomes.
Key Responsibilities:
- Conduct comprehensive assessments of new residents to identify their needs and develop personalized care plans.
- Collaborate with the multidisciplinary team to ensure seamless admissions and discharge planning.
- Coordinate external reviews, such as care reviews, medication reviews, and mental health reviews, to ensure residents receive the best possible care.
- Arrange healthcare appointments for residents, including GP, dental, and eye tests, and attend appointments for complex cases.
- Act as a key worker for nominated residents, developing and implementing care plans and risk assessments, and conducting regular key worker sessions.
- Monitor and follow up on care plans to ensure they are being implemented effectively.
- Conduct regular reviews of residents' needs and update records accordingly, using the Recovery Star Model.
- Participate in mandatory training courses to maintain clinical skills and knowledge.
- Ensure compliance with company policies, procedures, and systems, and promote a culture of excellence in care.
- Communicate effectively with residents, colleagues, and the multidisciplinary team to ensure the best possible outcomes.
- Provide supervision and guidance to junior staff, ensuring they adhere to policies and procedures.
- Exercise leadership and influence within the service, promoting effective communication, organization, and relationships.
- Ensure compliance with regulatory requirements, including those of the Care Quality Commission and the Nursing and Midwifery Council.
Requirements:

You will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
