As a Senior Social Worker in the Adult Assessment and Safeguarding Team, you will play a vital role in ensuring the safety and well-being of vulnerable adults.
Key Responsibilities:- Conduct comprehensive assessments under the Care Act 2014 to identify needs, risks, and eligibility for services.
- Identify and address safeguarding concerns, ensuring timely intervention to protect adults from abuse, neglect, or exploitation.
- Develop and implement person-centered care plans that promote independence, enhance well-being, and ensure safety.
- Collaborate with health professionals, housing services, community organisations, and other stakeholders to provide coordinated, multi-agency responses.
- Lead safeguarding investigations, implementing appropriate protection plans and following up to ensure ongoing safety and risk management.
- Regularly review and update care and safeguarding plans to adapt to any changing needs or circumstances.
- Advocate for the rights, well-being, and dignity of vulnerable adults, ensuring they have access to necessary services, resources, and support networks.
