Job Title: Social Worker - Children with Disabilities

We are seeking an experienced Social Worker to join our CWD Team, working with children and young people with disabilities. As a Social Worker, you will be responsible for providing support and guidance to families, completing assessments, and developing care plans.

Key Responsibilities:

  • Conduct assessments and develop care plans for children and young people with disabilities
  • Provide support and guidance to families, including case holding and case management
  • Work with multi-agency teams, including health and education professionals, to ensure comprehensive support for children and young people
  • Attend meetings and reviews, including Child in Need Visits, CLA reviews, and EHCP meetings
  • Carry out 12-month reassessments of Child and Family Assessments
  • Participate in safeguarding responsibilities, including S47 enquiries and Child Protection planning

Requirements:

  • HCPC registration
  • Relevant social work qualification with at least 2 years post-qualifying experience
  • Current DBS/DBS Update Service
  • Access to a car and a full UK driving licence
