Job Summary

Lakeridge Health is seeking a skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the delivery of exceptional healthcare services.

Key Responsibilities
  • Provide Patient Care: Use the nursing process to deliver patient-centered care, ensuring the physical, emotional, and psychological needs of patients are met.
  • Collaborate with Healthcare Team: Work effectively with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Maintain Competence: Stay up-to-date with the latest nursing practices, technologies, and evidence-based guidelines to maintain professional competence and provide high-quality care.
  • Document Patient Records: Accurately and thoroughly document patient information, including medical history, treatment plans, and outcomes.
  • Travel between Sites: Travel between Lakeridge Health sites as required to provide care and support to patients.
  • Perform Other Duties: Participate in other related duties as assigned by management, ensuring the delivery of exceptional patient care and services.
Qualifications
  • Education: Registered Practical Nurse Diploma from an accredited community college.
  • Registration: Registration with the College of Nurses of Ontario (in good standing).
  • Certifications: Current B.C.L.S. and Certificate in Medication Administration.
  • Experience: Experience caring for Nephrology patients and evidence of continuing education related to the requirements of the position.
  • Skills: Effective organizational, interpersonal, problem-solving, and teaching skills, with the ability to prioritize work and communicate effectively with patients, families, physicians, and colleagues.
  • Physical Demands: Physically capable of performing the duties of the position, including lifting, turning, and transferring patients manually and with mechanical aids.
  • Performance Record: Satisfactory performance record and demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork, and Joy.
  • Safety Programs: Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area.
Conditions of Employment
  • Collective Agreement: The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).
  • Criminal Investigation Reference: Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
