Job Title: Intensive Support Service Team Leader

Job Summary:

We are seeking an experienced and skilled professional to join our team as the Intensive Support Service Team Leader. The successful candidate will be responsible for leading a team of professionals in providing intensive support services to individuals with learning disabilities and complex needs.

Main Responsibilities:

  • Lead a team of professionals in providing intensive support services to individuals with learning disabilities and complex needs.
  • Develop and implement care plans that meet the individual needs of service users.
  • Collaborate with other healthcare professionals to ensure seamless care delivery.
  • Manage and coordinate staff to ensure effective service delivery.
  • Monitor and evaluate service delivery to ensure quality and effectiveness.
