We are seeking two experienced Social Workers to join our newly created Learning Disability and Autism Team at Tameside Council. As a Social Worker in this team, you will play a vital role in supporting adults with learning disabilities and/or autism, providing high-quality care and services that promote independence and wellbeing.
Key Responsibilities:
- Manage a caseload of adults with learning disabilities and/or autism, providing person-centered support and care.
- Conduct Care Act Assessments, Support Plans, Safeguarding, Mental Capacity Assessments, and Best Interest Decisions.
- Work collaboratively with other disciplines and local health and social care services to ensure a coordinated response to individuals and families.
- Develop and maintain effective relationships with service users, families, and carers.
- Participate in a duty rota and contribute to the team's overall objectives.
About You
To be successful in this role, you will need to be a qualified Social Worker with professional registration with Social Work England. You will have excellent communication skills, the ability to build effective professional relationships, and a passion for supporting adults with learning disabilities and/or autism.
