Position Overview

We are thrilled to announce an opportunity to become a vital member of our Critical Care Outreach Services.

This team operates continuously, providing essential support around the clock. The position offers a fulfilling experience in overseeing the care of patients who are acutely unwell or showing signs of deterioration within the hospital setting.

We seek candidates with substantial experience in Critical Care nursing, particularly those who are passionate about outreach and the management of acute health issues.

The responsibilities will encompass direct patient care, assisting ward staff in both managing patients and educating them on recognizing signs of deterioration. Additionally, the role includes collecting data to aid the Trust in adhering to NICE guidelines for acute illness management and overseeing the implementation of the National Early Warning Score (2) within electronic observation systems.

Key Responsibilities

Successful applicants will demonstrate enthusiasm for Outreach, assertiveness as clinicians, and a proactive mindset, coupled with advanced skills in assessing, identifying, and managing acute health conditions beyond the confines of critical care. Candidates must exhibit confidence and the ability to thrive in a demanding environment with a significant degree of autonomy. The position involves 12-hour shifts, covering both day and night schedules, including some weekend work, necessitating a flexible approach to scheduling.

  • Adhere to the NMC Code of Professional Conduct and relevant professional standards as the designated nurse for a specific group of patients.
  • Assess care and health education requirements.
  • Develop, implement, and evaluate care programs, including discharge planning for each patient.
  • Contribute to the evaluation of nursing practices and the integration of research and best practices, with a focus on acute illness management and the incorporation of the National Early Warning Score into electronic observation systems.
  • Participate in data collection to monitor service efficiency and enhance patient outcomes, including patient experiences.

Essential

  • Degree-level education or equivalent.
  • Completion of a Critical Care Module.

Desirable

  • Completion of PACR/PADRAP course.

  • Minimum of 2 years Band 5 experience.
  • Significant experience in a critical care environment.

Desirable

  • Recent Band 6 experience in critical care.
