Job Summary
The Dorset Clinical Commissioning Group is seeking a highly skilled and experienced Physiotherapist Specialist (Rotational) Frailty to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to patients with frailty, working closely with other healthcare professionals to ensure seamless care pathways.
Key Responsibilities
- Develop and implement individualized care plans for patients with frailty, taking into account their unique needs and goals.
- Conduct comprehensive assessments of patients' physical and functional abilities, identifying areas for improvement and developing strategies to address these.
- Provide evidence-based interventions to improve patients' mobility, strength, and functional ability, including exercise programs and education on self-management techniques.
- Collaborate with other healthcare professionals, including doctors, nurses, and therapists, to ensure comprehensive care and coordination of services.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve outcomes.
- Contribute to the development and implementation of policies and procedures related to frailty care, ensuring that they are evidence-based and aligned with best practices.
- Stay up-to-date with the latest research and guidelines on frailty care, applying this knowledge to improve patient outcomes and inform practice.
Requirements
- Eligibility for registration with the General Medical Council (GMC) or equivalent regulatory body.
- Basic medical qualification and completion of foundation training.
- MRCS or equivalent Part A qualification.
- Experience working in a multidisciplinary team and providing care to patients with complex needs.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to prioritize workload and manage time effectively, with a focus on delivering high-quality care.
Desirable Criteria
- Experience working in a clinical governance role, with a focus on quality improvement and patient safety.
- Knowledge of research methods and experience conducting research studies.
- Ability to teach and mentor junior colleagues, with a focus on developing their skills and knowledge.
