Job Summary

The Dorset Clinical Commissioning Group is seeking a highly skilled and experienced Physiotherapist Specialist (Rotational) Frailty to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to patients with frailty, working closely with other healthcare professionals to ensure seamless care pathways.

Key Responsibilities

  • Develop and implement individualized care plans for patients with frailty, taking into account their unique needs and goals.
  • Conduct comprehensive assessments of patients' physical and functional abilities, identifying areas for improvement and developing strategies to address these.
  • Provide evidence-based interventions to improve patients' mobility, strength, and functional ability, including exercise programs and education on self-management techniques.
  • Collaborate with other healthcare professionals, including doctors, nurses, and therapists, to ensure comprehensive care and coordination of services.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve outcomes.
  • Contribute to the development and implementation of policies and procedures related to frailty care, ensuring that they are evidence-based and aligned with best practices.
  • Stay up-to-date with the latest research and guidelines on frailty care, applying this knowledge to improve patient outcomes and inform practice.

Requirements

  • Eligibility for registration with the General Medical Council (GMC) or equivalent regulatory body.
  • Basic medical qualification and completion of foundation training.
  • MRCS or equivalent Part A qualification.
  • Experience working in a multidisciplinary team and providing care to patients with complex needs.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to prioritize workload and manage time effectively, with a focus on delivering high-quality care.

Desirable Criteria

  • Experience working in a clinical governance role, with a focus on quality improvement and patient safety.
  • Knowledge of research methods and experience conducting research studies.
  • Ability to teach and mentor junior colleagues, with a focus on developing their skills and knowledge.
