About the Role

We are seeking an experienced and passionate Dementia Clinical Nurse Specialist to join our team at Homerton Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients with dementia and their families.

Key Responsibilities
  • Provide clinical leadership on dementia care across the acute hospital site, working closely with the Lead Nurse for Dementia.
  • Manage referrals, assess patient needs, and allocate team members to provide personalized care.
  • Collaborate with healthcare teams, carers, and community partners to improve dementia care services.
  • Develop and implement evidence-based practices to meet national standards and guidelines.
  • Provide expert advice and support to professionals involved in dementia care, contributing to service improvement.

Person Specification
  • RGN or RMN with current NMC registration.
  • Evidence of training in dementia and delirium care.
  • Significant experience in acute hospital settings and dementia care.
