Job Title: SEND Casework Practitioner

We are seeking a highly skilled and dedicated SEND Casework Practitioner to join our team at Wokingham Borough Council. As a key member of our SEND team, you will play a vital role in supporting children and young people with Special Educational Needs and Disabilities (SEND) and their families.

Key Responsibilities:
  • Develop and maintain Education, Health and Care Plans (EHCPs) from initial application to annual review.
  • Work collaboratively with families, schools, and other key partner agencies to ensure children and young people receive appropriate provision that aligns with their holistic needs.
  • Engage with educational settings and relevant practitioners to support, advise, and monitor the local offer.
  • Manage a caseload of children and young people with SEND, prioritizing statutory processes for vulnerable individuals.
  • Stay up-to-date with SEN legislation and procedures, including relevant legislation such as the Children's Act and Code of Practice.
Requirements:
  • Experience of working with children and/or families with SEND.
  • Good IT skills, including proficiency in Microsoft Office and database systems.
  • Ability to travel to various locations within the borough to develop relationships with educational settings and attend training for key stakeholders.
