Job Description

We are seeking a highly skilled and experienced Social Worker to join our Community Mental Health Team in Widnes/Runcorn. As a key member of our team, you will be responsible for providing expert social work services to individuals and families, assessing and managing complex situations, and working collaboratively with other professionals to achieve positive outcomes.

Key Responsibilities:
  • Assess the need for social work services, including identifying risk and the need for protection, and determining the method of intervention to manage and minimize risk in complex situations.
  • Maintain and update accurate records of work undertaken, and carry out required administrative procedures.
  • Produce high-quality assessments and reports for a range of functions, including court proceedings and service planning.
  • Prepare for and attend supervision sessions, staff meetings, and events, and make use of all available training and developmental opportunities to enhance practice and knowledge.
  • Decide or advise on the use of appropriate social care and/or other services, resources, and contribute views on service provision to commissioners where applicable.
  • Comply with the statutory obligations of the council, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • Develop and provide training resources for social workers, support staff, and carers to enhance their knowledge and skills.
Requirements:
  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within an Integrated Hospital Discharge Team, with the ability to hit the ground running.
  • Ability to manage a complex caseload and make sound decisions quickly and effectively.
  • Excellent communication, assessment, and organizational skills, with a commitment to collaborative working.
  • Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • Proficiency in using IT systems, including knowledge of Eclipse (desirable).
  • Full UK driving license and access to a car for work purposes.
