Position Overview
The Pulmonary Rehabilitation team operates within the community care division of County Durham & Darlington NHS Foundation Trust, dedicated to enhancing community health services and empowering individuals to maintain independence and quality of life. Our established pulmonary rehabilitation program serves patients across the region, focusing on improving health outcomes for those with chronic respiratory conditions.
We are currently seeking a Band 3 Pulmonary Rehabilitation Assistant to join our expanding community physiotherapy services. This role is a permanent part-time position, primarily located at various healthcare facilities within the trust. Flexibility to work across multiple sites is essential.
Key Responsibilities
The primary function of this role is to assist in the implementation of a thorough community pulmonary rehabilitation exercise regimen for patients diagnosed with COPD. The program aims to:
- Promote pulmonary rehabilitation as a vital aspect of managing chronic respiratory conditions.
- Enhance patients' confidence in self-managing their health.
- Improve overall health-related quality of life, including breathlessness management and exercise capacity.
- Reduce disability associated with chronic respiratory diseases.
Duties Include:
- Supporting qualified physiotherapists in assessing patient eligibility for the program.
- Conducting objective assessments.
- Assisting in the delivery of both individual and group exercise sessions, utilizing both digital platforms and in-person interactions.
- Providing educational components of the rehabilitation program.
- Aiding in the data collection and input for national respiratory audits.
- Managing equipment orders and performing administrative tasks, including scheduling appointments and processing referrals.
Collaboration with general practitioners, primary care networks, clinical commissioning groups, and secondary care respiratory teams is crucial for fostering strong professional relationships.
Essential Qualifications and Skills
- A valid driving license and willingness to travel between sites as required.
- Proficiency in IT, with experience in System 1 being advantageous.
- Previous experience in delivering rehabilitation programs within community settings.
Desirable Attributes
- Recent experience in pulmonary rehabilitation within the NHS.
- Competency in mobility assessments and exercise progression.
- NVQ Level III in a healthcare-related field or equivalent.
