Social Care Professional

We are seeking a highly skilled and dedicated Adult Social Care Practitioner to join our Gloucester Locality Team. As a key member of our team, you will be responsible for undertaking assessments and support plans with individuals who have physical disabilities or other complex needs.
Our team works collaboratively with families, carers, and other professionals, agencies, and organizations across Gloucestershire to promote independence and reduce the need for hospital admission or long-term care.
The ideal candidate will have experience working in a health and/or social care setting, knowledge of the Care Act, and the ability to apply it in practice. You will also have excellent interpersonal skills, good verbal and written communication, and good IT skills.
About YouTo be successful in this role, you will need to have:
- Relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience
- Training related to Health and Social Care assessments
- Experience of managing a caseload, including working with individuals with a range of physical disabilities, medical, mental health difficulties, and learning difficulties
- Evidence of good interpersonal skills, good verbal and written communication
- Good IT skills
