Job Title: Advanced Health Improvement Practitioner

£29,969 - £36,483 a year - Fixed term contract

Job Summary:

We are seeking an experienced Advanced Health Improvement Practitioner to join our team at Livewell Southwest. The successful candidate will be responsible for delivering specialist interventions for behaviour change, working with partners to reach target populations, and actively seeking out hard-to-reach communities.

Main Responsibilities:

  1. Design and deliver one-to-one and group interventions to promote health and wellbeing.
  2. Liaise with external partners to develop and implement health improvement initiatives.
  3. Identify and engage with hard-to-reach populations to promote health and wellbeing.
  4. Develop and deliver training programmes to support the health and wellbeing of Livewell Southwest teams.
  5. Generate income through training programmes and explore other areas for income generation.
  6. Contribute to the development of the communications and marketing strategy.
  7. Develop resources and deliver training programmes to support the health and wellbeing of the community.
  8. Act as a Health Champion, making every contact count.

Requirements:

  1. Induction of new team members.
  2. Line management of junior staff, including supervision and appraisal.
  3. Ensuring clinics run smoothly.
  4. Delivery of training to groups.
  5. Collect data regarding purchasing and invoicing to support budgetary requirements.
  6. Ensure records are up-to-date and stored as per record keeping policy.
  7. Provide advice and support to the public over various communication methods.
  8. Contribute to service development and implement changes to established processes.

Working Relationships:

The post holder will interact with the general public, work colleagues, external stakeholders, and Livewell Southwest employees at various venues.

Physical Demands of the Job:

This role involves frequent sitting and standing, manual handling of equipment, and keyboard work. The post holder will be required to maintain competencies relevant to the role.
