An exceptional opportunity has emerged for a dedicated General Practitioner to become a pivotal member of the leadership team focused on enhancing the Frailty Pathway for patients. This role is designed to bridge the gap between Primary and Secondary care, allowing you to impact patient care on a broader scale.
Flexible Work Arrangement
This position offers flexibility, making it suitable for those balancing other commitments or pursuing a portfolio career. The role will primarily involve virtual meetings and electronic communication, with a commitment of 24 hours per week (3 days / 6 sessions).
Key Responsibilities
- Assessing and refining existing care pathways while developing innovative solutions
- Evaluating clinical guidelines to ensure best practices
- Supporting our Virtual Community Wards initiative
- Providing educational resources and training
- Engaging in quality improvement initiatives
- Participating in local, regional, and national forums to contribute to broader strategic goals
