Job Summary

We are seeking a skilled and compassionate Healthcare Assistant to join our multi-professional team providing palliative nursing care interventions for patients and their families in the community. As a Healthcare Assistant, you will work under the supervision of a Team Leader to provide care and support to patients in their homes, promoting their independence and dignity.

Main Responsibilities
  • Provide high-quality care and support to patients in their homes, in accordance with their individual needs and management plans.
  • Work collaboratively with the multi-professional team to deliver comprehensive care and support to patients and their families.
  • Contribute to the development and implementation of care plans, ensuring that patients receive the best possible care and support.
  • Maintain accurate and up-to-date records of patient care and progress.
  • Participate in ongoing training and professional development to enhance your skills and knowledge.

Person Specification

We are looking for a skilled and compassionate Healthcare Assistant with:

A Level 3 Diploma in Health and Social Care (or equivalent).Recent experience in a healthcare setting, preferably in general or community nursing.Excellent communication and interpersonal skills.Ability to work autonomously and as part of a team.Good record-keeping skills and attention to detail.A valid UK driving license and access to a car.
