About the Role

We are seeking a highly skilled and experienced Senior Adult Nurse Case Manager to join our team at Leaders in Care. As a key member of our community care team, you will be responsible for providing high-quality care and support to adult clients and their families in the South West London area.

Key Responsibilities
  • Manage a reduced case load of service users, providing personalized care plans and delivering nursing interventions as needed.
  • Support the business with growth plans by mentoring Adult Nurse Case Managers and contributing to the development of new care packages.
  • Effectively manage the multidisciplinary team (MDT) through strong communication and teamwork, ensuring seamless care delivery.
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload.
  • Deliver nursing interventions, including the management of medicine delivery, and build and maintain sophisticated relationships with clients' families.
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.
Requirements
  • RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators within the last 2 years.
  • Supportive management style that can bring out the best in others, with evidence of effective leadership and management of HCAs.
  • Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT.
  • Suitable for Band 5 / Band 6 Nurses with a full UK Driving Licence desirable but not essential.
Benefits
