About the Role

As a Social Worker, you will be responsible for undertaking annual statutory reviews, unplanned reviews, and working in partnership with allied Adult Social Work Teams to provide support to those who need it most. You will work creatively with individuals, local communities, and our partners to empower them to deliver the best possible outcomes for people and create a positive Adult Social Care culture.

Key Responsibilities
  • Provide high-quality social work practice, ensuring individuals have a solution-focused plan towards achieving their goals and maintaining their wellbeing.
  • Work creatively with individuals, local communities, and our partners to empower them to deliver the best possible outcomes for people.
  • Undertake annual statutory reviews, unplanned reviews, and work in partnership with allied Adult Social Work Teams.
  • Develop and maintain effective working relationships with a range of partners, colleagues, adults, families, and their carers.
