As a Medication Management Specialist, you will play a vital role within the general practice team.
This position will involve working under the guidance of Clinical Pharmacists to guarantee the safe, precise, and timely delivery of prescribed medications to patients.
You will also provide both technical and administrative assistance to the Clinical Pharmacists and healthcare providers.
Your responsibilities will include addressing medication-related inquiries from patients and conducting safety audits regarding medication prescribing, with findings shared with the clinical team.
Candidates must possess a valid registration as a pharmacy technician with the General Pharmaceutical Council (GPhC) and hold a Diploma Level 3 or an equivalent qualification in pharmacy services.

Primary Duties
The Medication Management Specialist will undertake the following key responsibilities:
- Engage with patients to ensure effective medication use through collaborative discussions.
- Perform medication optimization tasks, including administration checks and support for medication reviews.
- Assist with medication reviews and reconciliation for new care home patients, ensuring smooth transitions between care settings.
- Provide expert advice on public health and social care needs, including lifestyle recommendations.
- Maintain involvement in shared care protocols and collaborate with specialist pharmacists for complex cases.
- Support initiatives aimed at reducing inappropriate antibiotic prescribing.
- Contribute to the delivery of medication optimization and patient safety audits.
- Implement national prescribing policies and guidelines within primary care settings.
- Participate in annual performance reviews with your manager to set and assess training objectives.
- Contribute to public health initiatives through direct care or advisory roles.
- Ensure a clean and organized working environment at all times.
Technical and Administrative Duties
The technical and administrative responsibilities of the Medication Management Specialist include:
- Collaborate with the Clinical Pharmacist team to enhance medication optimization processes.
- Assist practice reception teams in managing prescription requests efficiently.
- Lead initiatives to maximize the value of medications through electronic prescriptions and safe prescribing systems.
- Provide training on the legal and secure handling of medications.
- Foster relationships with other pharmacy professionals to support integrated care.
- Assist in the implementation of quality improvement initiatives.
- Undertake tasks consistent with the role's scope and responsibilities.
- Be adaptable to changes in duties as required by workload and staffing levels.
Qualifications
Essential Qualifications
- Registered with the General Pharmaceutical Council (GPhC) and possess a Diploma Level 3 or equivalent.
- Meet the qualification and training criteria set by the GPhC for Pharmacy Technicians.
- Enrolled in or completed an approved training pathway.
Experience Requirements
- Experience working under clinical supervision to ensure safe medication use.
- Familiarity with primary care settings.
- Understanding of healthcare provision in GP practices and related services.
