Job Summary

We are seeking an experienced Practice Nurse to join our bank team, working across our Enhanced Access Services delivered on behalf of Eccles and Irlam PCN at our clinical hub.

This is a bank role, ideal for an individual who wants flexibility in their working hours and can cover clinics outside of core hours, including evenings and weekends.

Main Duties

  • Undertake cytology clinics, chronic disease management, and immunisations
  • Manage the care of patients with long-term conditions, assessing, planning, implementing, and evaluating treatment plans
  • Assess patient needs, providing appropriate care and treatment, and referring to colleagues or other agencies as necessary
  • Make every contact count, offering support and advice on improving health and wellbeing
  • Maintain accurate and contemporaneous records, ensuring patient notes are fit for purpose
  • Support the provision of EPIC Services and promote the fulfilment of the rota
  • Monitor stock levels and ensure stock is ordered and maintained as required

Communications & Leadership

  • Attend departmental meetings as required
  • Ensure effective communication with patients and staff
  • Provide reassurance to patients, gaining consent using empathy and persuasive skills
  • Communicate with patients with learning or language disabilities
  • Highlight areas of concern or improvement to line management

Policy and Procedures

  • Take reasonable care of your own health, safety, and wellbeing, and that of your colleagues
  • Uphold the confidentiality of all records held by SPCT
  • Comply with all SPCT policies and procedures, identifying improvements where necessary
  • Promote the application of standards, protocols, and codes of practice as set by the Royal College of Nursing and other regulatory bodies
  • Understand how to use the Ulysses reporting system and the Incident Reporting Policy and procedures

  • Safeguarding is everyone's responsibility; employees must have regard for the need to safeguard and promote the welfare of children and adults at risk

Person Specification

Qualifications

  • Registered with the Nursing and Midwifery Council
  • Certified Cytology Sample Taker
  • Current Level 3 Safeguarding for Adults & Children Certificate
  • Current Basic Life Support Certificate

Experience

  • Experience of supporting patients with long-term conditions
  • Experience of undertaking a wide range of nursing services, including venepuncture, ECG measurement, and health checks
  • Experience of working in a primary care environment
  • Competent in the use of patient record management systems such as EMIS
  • Flexible approach to working hours to meet the needs of the service
