Advanced Nurse Practitioner

We are seeking a highly skilled and motivated Advanced Nurse Practitioner to fill a key role in our organization. As an Advanced Nurse Practitioner, you will play a vital role in providing high-quality patient care and contributing to the development of our practice.
Key Responsibilities- Provide clinical assessment, diagnosis, and treatment for adult and pediatric patients, utilizing evidence-based practice guidelines and protocols.
- Work collaboratively with healthcare professionals to deliver patient-centered care, ensuring seamless transitions and optimal outcomes.
- Develop and implement care plans in collaboration with patients, families, and healthcare professionals, taking into account individual needs and preferences.
- Contribute to the identification of patients at risk of emergency hospital admissions, and develop strategies to mitigate these risks.
- Support patients in adopting healthy lifestyles and promoting self-care, through education and counseling.
- Work with General Practitioners to manage patients with urgent requests and needs, ensuring timely and effective care.
- Deliver care coordination to patients identified as at-risk, providing proactive support and interventions.
- Manage the process of clinical care coordination, including surgery consultations and home visits, ensuring seamless communication and collaboration.
- Contribute to the development of an integrated approach to patient care, aligning with organizational goals and objectives.
- Registered Nurse with Nursing and Midwifery Council registration.
- Masters certificate, diploma, or full Masters in Advanced Clinical Practice.
- Qualified Independent Nurse Prescriber on the NMC register.
- Experience of working in a primary care environment, with a strong understanding of local healthcare systems and policies.
- Excellent interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare professionals.
