Job Summary:
We are looking for a highly motivated and experienced Practice Nurse to provide exceptional nursing care to our patients. The ideal candidate will possess excellent communication skills, be able to work autonomously, and demonstrate a commitment to professional development.
Main Responsibilities:
- Provide routine patient care, including administering vaccinations and conducting health promotion programs.
- Manage wound care and chronic disease management, including assessing patient needs and implementing treatment plans.
- Support Infection Prevention Control and maintain accurate patient records.
- Develop and implement care management programs, including health promotion activities and vaccination programs.
- Prescribe medication and coordinate travel medicine services as needed.
- Lead NHS Health Clinics and recognize patients with mental health needs.
- Support patients with medication use and liaise with external services as required.
- Deliver health promotion activities and support safeguarding initiatives.
Requirements:
- Registered Nurse (NMC) qualification and primary care experience.
- Chronic disease management experience and autonomous work experience.
- Infection control knowledge and excellent communication skills.
- Ability to work in a team and demonstrate good time management and problem-solving skills.
- Commitment to professional development and motivated individual.
Desirable Qualifications:
- Mentor qualification and prescribing qualification.
- Experience in community nursing and clinical governance knowledge.
- Public health awareness and motivated and forward-thinking individual.
