Key Responsibilities:

  • Provide exceptional patient care, ensuring the physical, emotional, and social needs of our residents are met.
  • Develop and implement individualized care plans, working closely with multidisciplinary teams to achieve optimal outcomes.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions.
  • Collaborate with colleagues to ensure seamless care transitions and maintain a safe and supportive environment.

Requirements:

  • Valid NMC registration and a current pin number.
  • Excellent knowledge of clinical practices, including infection control, moving and handling, wound care, and drug administration.
  • Proven experience in a care home setting, with a focus on elderly and dementia care.
  • Strong communication and interpersonal skills, with the ability to work effectively with colleagues, residents, and families.
