Position Overview

The Clinical Nurse Specialist will play a pivotal role in the management and care of patients with chronic conditions, utilizing evidence-based methodologies to enhance patient outcomes. This position involves collaboration with a multidisciplinary team to ensure that nursing services are effectively provided to the designated patient demographic.

Key Responsibilities

The Clinical Nurse Specialist will oversee various clinical functions, including but not limited to:

  • Supporting the Infection Prevention Control (IPC) lead and adhering to IPC policies through audits and training.
  • Contributing to the development of the IPC annual report in collaboration with management.
  • Creating and implementing a comprehensive care management program for vulnerable and housebound patients.
  • Assessing patient needs to ensure the delivery of appropriate healthcare, grounded in evidence-based practices.
  • Designing and executing health promotion initiatives aimed at improving overall well-being.
  • Formulating and evaluating individualized treatment plans for patients with chronic diseases to foster health and wellness.
  • Identifying and managing patients at risk of developing long-term health issues, thereby preventing negative health outcomes.
  • Providing routine nursing care in accordance with clinical guidelines and best practices.
  • Diagnosing and managing both acute and chronic health conditions as necessary.
  • Prescribing medications within the scope of practice and established guidelines.
  • Reviewing patient medications to enhance adherence and understanding.
  • Coordinating travel medicine services for patients.
  • Requesting and processing pathology services as required.
  • Overseeing chronic disease management clinics and facilitating referrals to specialized care when needed.
  • Maintaining accurate clinical documentation in compliance with relevant legislation.
  • Ensuring effective use of clinical coding by the nursing team.
  • Managing chronic disease registers to ensure up-to-date patient information.
  • Participating in well-woman and well-man clinics to promote health.
  • Adhering to the practice chaperone policy during patient interactions.
  • Assisting physicians with minor surgical procedures as required.
  • Prioritizing health concerns and intervening appropriately.
  • Supporting the clinical team in emergency situations.
  • Recognizing and referring patients with mental health needs.
  • Implementing vaccination programs for both adults and children.
  • Guiding patients in the use of prescribed and over-the-counter medications.
  • Liaising with external agencies to ensure comprehensive support for vulnerable patients.
  • Delegating clinical tasks appropriately while ensuring safe practices.
  • Assisting the clinical team with safeguarding matters in line with local and national policies.
  • Understanding and applying local policies regarding substance abuse and addiction.
  • Delivering opportunistic health promotion when appropriate.

Additional Responsibilities

Beyond primary duties, the Clinical Nurse Specialist may also be involved in:

  • Developing and reviewing audit protocols to enhance practice efficiency.
  • Creating and updating nursing protocols in collaboration with practice partners.
  • Providing mentorship and guidance to nursing team members.
  • Engaging in local initiatives to improve service delivery and patient care.
  • Participating in shared learning opportunities to enhance patient outcomes.
  • Continuously reviewing clinical practices in response to national policies.
  • Participating in the analysis of significant events to improve practice safety.
  • Coordinating the nursing team schedule to ensure adequate staffing.
  • Leading the implementation of service enhancements within the nursing team.
  • Supporting the practice team in policy and protocol reviews to ensure compliance.

Qualifications and Experience
