About the Role

The Care Coordinator will work within the Primary Care Network (PCN) team, providing coordination, administrative, and support to the clinical teams and other members of the network. The post holder will be primarily responsible for coordinating the PCN response to the Directed Enhanced Service requirements across PCN GP practices.

Main Responsibilities

The Care Coordinator will lead project work and collate practice data, identifying and understanding patient cohorts, patient engagement, and administrative tasks that support the smooth and effective running of the PCN. The administrative support to the primary care network will include activities such as organizing meetings, recording minutes, and action. The post holder will need to have excellent telephone communication to support our patients and also external providers such as care homes and safeguarding teams.

Requirements

  • NVQ Level 3, advanced level, or equivalent qualifications or working towards or relevant work experience
  • Excellent IT skills, able to work in a fast-paced environment, and work well under pressure
  • Enthusiastic and able to work as a team, using own initiative
  • Experience with SystmOne, or a good understanding of healthcare, with full training and development provided
